HOW TO USE ON-LINE REQUEST :
In order to utilize this on-line service you must click on the appropriate links below. Please note that after doing so you will be given a Security Warning advising you that the information you are submitting is secure. In order to proceed you must answer yes to the question; do you want to proceed ? You will than be directed to the corresponding form to complete your request.
HOW TO MAKE YOUR APPOINMENT BY PHONE :
You must dial our local coustumer service line : 305-485-1111 or dial our toll free number : 1-855-732-5263 our freindly customer service staff will coordinate de visit.
MEDICATION REFILLS :
For your convenience we have a on-site pharmacy or you may utilze another pharmacy of your choice. If you choose to utilize off-site pharmacies please contact us a minimum of 72 hours prior to the time you intend to pick-up your medication. You can iether request a refill of your medications by phone calling us at : 305-485-1111 during normal hours of operation. We also suggest you contact the pharmacy prior to going to pick-up the medications to be certain your order is waiting for you. Sorry medications refills are not performed after hours or on weekends so we ask that you plan accordingly.
ARRANGING REFERRALS TO SPECIALISTS :
We understand the importance of having your referrals ready for your prior to your appoinments with specialist. We ask that you adhere to the fallowing policy as it relates to referrals. All referral requests are handled by our referral coordinator. Routine referrals are obtained every Friday. We ask that you request referrals a minimum of 1 week prior to the scheduled appoinment. You may contact our referral coordinator to arrange a referral by calling our central office at : 305-485-1111 between 9:30 AM and 2:00 PM . All requests for urgent referrals should be made by telephone and reviewed by our Doctor’s.
Everything possible will be done to obtain urgent referrals as quickly as possible.
OBTAINING LABORATORY / DIAGNOSTIC TEST RESULTS :
Most often it best for you to schedule a fallow up appoinment within 2 to 14 days from your visit to discuss your results with the doctor or physician assistant in our office. Some test take one day to be completed and others such as pap smears and biopsies requiere up to 14 days. The doctor or medical assistant can best advise you at the time the test are obtained as to when to come back in for your results.
Call our office at 305-485-1111 and leave a voicemail message for one of our medical assistants and they will do their best to return you call within next 24 hours. Due the volume of calls we receive daily, we would greatly appreciate you using this method as a last resort.
